Guide 65 ("we," "us," or "our") is a registered DBA (Doing Business As) of Unity Benefits Group LLC, a South Carolina limited liability company and licensed insurance agency. These Terms of Service ("Terms") govern your use of guide-65.com and our services. By accessing our website or submitting a form, you agree to these Terms.

1.Our Services

We provide Medicare plan guidance and enrollment assistance, including help comparing Medicare Advantage, Medicare Supplement, and Part D prescription drug plans, as well as related health and life insurance products. This is a solicitation for insurance. A licensed insurance agent may contact you.

2.No Government Affiliation

We are not connected with or endorsed by the U.S. government or the federal Medicare program. We do not offer every plan available in your area. Any information we provide is limited to the plans we do offer in your area. To learn about all of your options, contact Medicare.gov or call 1-800-MEDICARE (1-800-633-4227).

3.SMS Program, Messaging Use Cases & Consent

SMS program. If you provide your phone number and opt in, you consent to receive calls and text messages from Guide 65 (a DBA of Unity Benefits Group LLC), which may be sent using automated technology. This is a recurring messaging program for individuals interested in Medicare and related insurance.

Messaging use cases. Messages you may receive include: appointment confirmations and reminders, plan-review and quote follow-ups, Medicare enrollment-period reminders, customer-service responses, account and plan-document updates, and occasional promotional offers and educational content.

Frequency & rates. Message frequency varies based on your interaction with our services. Message and data rates may apply.

Opt-out & help. Consent is not a condition of purchase. You may opt out of text messages at any time by replying STOP; reply HELP for help, or contact us at the details in Section 9.

Carrier liability. Mobile carriers are not liable for delayed or undelivered messages. Supported carriers may change without notice.

4.Eligibility

You must be at least 18 years old and provide accurate information that belongs to you in order to use our services and submit forms on our website.

5.Intellectual Property

All content on guide-65.com, including text, graphics, logos, and the Guide 65 brand, is the property of Unity Benefits Group LLC or its licensors and is protected by applicable law. You may not reproduce or distribute it without permission.

6.Disclaimers & Limitation of Liability

Our website and services are provided "as is" without warranties of any kind. To the fullest extent permitted by law, Unity Benefits Group LLC is not liable for any indirect, incidental, or consequential damages arising from your use of our website or services. Insurance products are offered only through licensed agents and are subject to applicable state regulations and carrier underwriting.

These Terms are governed by the laws of the State of South Carolina, without regard to its conflict-of-law principles.
